Dobutamine was administered intravenously by an infusion pump with an infusion rate of 10 g min 1 for 3 min, increasing by 10 g min 1 every 3 min up to a maximum of 40 g min 1. In patients not achieving 85% of their age- and genderpredicted maximal heart rate and without symptoms or signs of myocardial ischaemia, atropine was administered, starting with 025 mg intravenously and repeated up to a maximum of 10 mg within 4 min with continuation of dobutamine infusion. Throughout the dobutamine infusion the ECG was continuously monitored 3 leads ; and recorded 12 leads ; at 1 min intervals. Reasons for interruption of the test were: severe angina, severe and or extensive wall thickening abnormalities, symptomatic reduction in systolic blood pressure 40 mmHg from baseline, hypertension blood pressure 240 120 mmHg ; , significant tachyarrhythmias, horizontal or downsloping ST-segment depression more than 02 mV at interval of 80 ms after the J point compared with baseline, ST-segment elevation more than 01 mV in patients without previous myocardial infarction, and any serious side effect regarded as being due to dobutamine. Metoprolol was available to reverse the effects of dobutamine if patients did not revert spontaneously and quickly. The echocardiograms were recorded on videotape and digitized on optical disk Vingmed Sound, Horten, Norway ; . Echocardiographic analysis of the left ventricular wall was performed according to the standardized 16-segments model[10]. Systolic wall thickening and or wall motion ; was visually evaluated, and each segment was graded with a 5-point scoring model 1 normal; 2 mild hypokinesis; 3 severe hypokinesis; 4 akinesis; and 5 dyskinesis ; during any dobutamine or atropine ; stage. Dobutamineatropine stress echocardiography was considered non-diagnostic when peak heart rate was 85% of maximal in the absence of new or worsening wall thickening abnormalities. Dobutamineatropine stress echocardiography was considered positive in patients with new or worsening wall thickening abnormalities at any dobutamine or atropine ; stage in d1 segment. Positive dobutamineatropine stress echocardiography studies were further stratified into high-risk and low-risk studies. High-risk studies were defined as studies in which new or worsening wall thickening abnormalities.
At one month, the mortality rate was halved among patients treated with levosimendan compared to those treated with dobutamine 8% versus 17%; p 049; hazard ratio 43 with 95% ci of 18- 00 and dofetilide.

Preoperative dobutamine stress echocardiography
We report the case of a patient admitted to the emergency room of our hospital with acute myocardial infarction in the inferior wall, who underwent primary angioplasty, complicated by shock that did not respond to the administration of catecholamines and intra-aortic counterpulsation balloon. The use of transesophageal echocardiography was crucial in showing systolic anterior motion of the mitral valve obstructing the left ventricular outflow tract. Dynamic obstruction of the left ventricular outflow tract with systolic anterior motion of the mitral valve has been reported in association with several conditions, such as hypertensive concentric hypertrophy1, excessive sympathetic stimulation2, 3, pericardial tamponade4, dobutamine stress tests5, postoperative mitral valvuloplasty6, and aortic valve replacement7, in addition to obstructive hypertrophic cardiomyopathy. Some cases of dynamic left ventricular outflow tract obstruction complicating anterior myocardial infarction have been reported8-12; however, no case of inferior myocardial infarction has been reported in association with that entity. The hemodynamic findings of myocardial infarction aggravated by left ventricular outflow tract obstruction have some implications in the treatment; these implications are not common in the treatment of the infarction without that complication. In the situation of hemodynamic deterioration, the use of intra-aortic counterpulsation balloon may have a deleterious effect on the patient diagnosed with that obstruction and may hinder improvement in clinical condition13, 14. The use of vasodilators and some catecholamines may also worsen the state of shock 14. We report the case of a patient with inferior myocardial infarction associated with left ventricular outflow tract obstruction aggravated by the use of intra-aortic counterpulsation balloon and doral.
Whether measurement of cardiac output using the thermal dilution technique TDCO ; is valid in the presence of tricuspid regurgitation TR ; is controversial. We assessed the accuracy and precision of the technique in pigs by comparison with data from an electromagnetic flowmeter on the aorta EMCO ; .
