As we all know, Protopic Tacrolimus ; and a similar drug Elidel Pimecrolimus ; are the newest invention in topical immunomoderator drugs. They revolutionize the therapy of inflammatory dermatosis particularly those dermatosis with an immunological basis. The classic example is Atopic Dermatitis. Because of their immunomodulation effect, they are increasingly being used by dermatologists to treat other inflammatory or non inflammatory dermatosis with an immunological basis like Vitiligo. As time goes by, more knowledge about their clinical effect and or efficacy will be known. Those called "off label" use in conditions like Alopecia areata, Seborrheic dermatitis, Vitiligo, Lichen planus and other eczema etc. etc. If one wants to know about their use in other conditions, one can read all the latest dermatological journals or consult your dermatologist colleagues about how to use these drugs in the conditions listed above. The following represents only my own personal view as to how to use these drugs in day to day clinical practice: 1. Atopic Dermatitis: - I find both drugs i.e. Protopic ointment and Elidel cream quite effective provided the eczema is not too severe. If the eczema does not respond to them, one still has to use topical steroid cream or ointment, may be adding topical oral antobiotic if the eczema is secondarily infected. Once the dermatitis eczema is controlled by steroid, then I will switch to protopic or Elidel. If the patient and elmiron.

Recognize and limit emotional stress. Phototherapy is sometimes used to treat atopic dermatitis for children over 12 and adults. Ultraviolet A or B light or both can be used. Photochemotherapy combines ultraviolet light with a drug called psoralen. This therapy can cause premature aging of the skin and skin cancer. The Centers for Disease Control and Prevention recommends that people who have eczema should not take the smallpox vaccine unless they have been exposed to the smallpox virus. This includes anyone who has eczema even if the condition is not currently active, mild or was experienced as a child.

Arrival at Sarnath, the Buddha actually met the five ascetics. We were nearing our destination. Suddenly we saw above the tree-tops, about a mile away, the pinkish-grey pinnacle of the Mulagandhakuti Vihara, the new Sinhalese temple constructed about twenty years earlier. We were there! After a few hundred yards the road turned sharply to the right and as though in a dream we saw before us the park-like prospect of Sarnath. The next few days were among the pleasantest and the most painful of my entire existence. Since it was the height of the hot season, when pilgrims were few, we had the whole place practically to ourselves, and after the congestion and clamour of Benares, the spaciousness and peace of Sarnath, with its green lawns, flowering trees, and cool, well-kept shrines, was delectable indeed. Though we saw practically everything of interest, the main object of our attention was, of course, the polished granite column set up by the Emperor Ashoka to mark the exact spot where the Buddha taught the five ascetics. This column, or what was left of it, stood in a roofed-in enclosure of its own in the midst of several acres of ruined temples, monasteries, and votive stupas, all of which had been uncovered in the course of excavations, and were now carefully preserved. The famous lion capital by which it had originally been surmounted was kept in the museum. Yet beautiful as Sarnath was, I knew it had not always been so. Muslim invasion and orthodox Hindu revival had between them levelled it to the ground, and for hundreds of years the very name of the place was virtually unknown. At the end of the last century it was being used as a breeding ground for pigs. In the course of the last few decades, however, a great change had taken place, and with the establishment of temples and monasteries, and the provision of facilities for pilgrims, Sarnath had been restored to a modest semblance of its former glory. From my contact with the Maha Bodhi Society in Calcutta I knew that all this was due to the initiative of Anagarika Dharmapala, who had started there a branch of the Society and built the Mulagandhakuti Vihara. Dharmapala himself had died nearly twenty years before, but his work was being continued by Sinhalese monks, some of whom had been his personal disciples. It was from these monks that we were hoping to receive ordination. Rarely in the history of Buddhism can two candidates for admission to the Sangha have been more quickly or more cruelly disappointed. However, optical imaging methods suffer from very substantial attenuation of the light signal and, thus, are not amenable to applications in large animals and in patients. On the other hand, radionuclide imaging methods such as g-camera, SPECT, and PET have excellent depth sensitivity and can detect accumulation of gene expression within the transfected tumors anywhere in the body on the basis of gene expression imaging 22, 29, 30 ; . A major advantage of PET is the ability to generate quantitative highspatial resolution 3-dimensional images. When combined with other forms of tomographic imaging, such as CT or MRI, fusion images of functional and anatomic data will provide more detailed in situ information of marker genes' expression and localization. For the purpose of gene expression imaging, several transgenes could provide promising payloads for Sindbis vector tumor-specific delivery in vivo, including the HSVtk, dopamine-2 receptor 31 ; , or the sodium iodide symporter 32 ; . For all of these systems, specific tumor targeting is rapid as is image contrast from clearance over time of the unreacted probe from normal tissues. For HSVtk, there are a variety of choices of reporter probes that may be used as well, both purine- and pyrimidine-based nucleosides, with varying radiotracer half-life, such as 18F-FEAU 110-min half-life ; and 124I-FIAU 4-d half-life ; . FIAU labeled with 124I, in particular, can be imaged several days after injection, at a time when virtually all of the unreacted tracer will have been cleared from the body and image contrast is maximized. A detailed comparison of the probes has been reported 33 ; , and the pyrimidine-based tracers appear to have higher uptake than the purine ones. Also, the probe FIAU, when labeled with 124I, can be imaged several days after injection, at a time when virtually all of the unreacted tracer will have been cleared from the body. Thus, the choice of probe will depend on the distribution of tumor and the time required to achieve effective tumor contrast for imaging. In vivo transfection after parenteral adminstration of Sindbis vector appears to be very efficient, on the basis of a comparison of the percentage dose per gram uptake in the imaged tumors from the current study with reported uptake values in the literature 33 ; . For example, after intravenous injection, FIAU uptake was 1.5 %ID g, at 2 h after injection, in a tumor transfected in vitro 33 ; , compared with our results Fig. 6 ; of .3% dose per gram. Thus, in comparison with optimized in vitro transfection of tumor cells, there is a doubling of uptake after parenteral transfection of the Sindbis vector--this seems to us to indication of how efficient the biology of viral transfection can be in vivo under the right circumstances. Thus, our data demonstrate the feasibility of detecting HSVtk enzyme activity in tumors after systemic treatment of a GDEPT vector in living animals. Our findings set the and eligard.
